Onsite Visit: To initiate the assessment process, we will schedule an onsite visit at your preferred location, whether it's your loved one's home or a care facility. This allows us to conduct the assessment in a familiar and comfortable environment.

Functional and Cognitive Assessment: During the onsite visit, our experienced clinicians, part of GuideWISE, will perform a thorough functional and cognitive evaluation. This assessment may include:

  • Functional Assessment: This involves assessing your loved one's physical abilities, such as mobility, strength, and balance. We will also evaluate their daily living skills, including bathing, dressing, and meal preparation. 
  • Cognitive Assessment: Our clinicians will assess cognitive functions such as memory, attention, problem-solving, and language skills. This helps us understand your loved one's mental capabilities and any potential cognitive challenges. 

Written Report with Recommendations: Following the assessment, we will provide a detailed summary of the functional and cognitive assessment results, highlighting your loved one's strengths and areas that may require additional support. 

Personalized Recommendations: Our clinicians will offer personalized recommendations based on the assessment findings. These recommendations may include: 

  • Care Plan: We will outline a customized care plan that addresses your loved one's specific needs and preferences. This may include suggestions for in-home care, therapy, or assistive devices.
  • Medical Referrals: If necessary, we will recommend consultations with medical specialists, such as neurologists, geriatricians, or physical therapists, to address any identified health concerns. 
  • Therapeutic Interventions: We may suggest cognitive rehabilitation programs or therapies to enhance cognitive function and emotional well-being.
  • Safety Measures: To ensure your loved one's safety, we might propose modifications to their living environment, such as installing handrails or removing trip hazards.
